MEDICATION SAFETY (PART 3)

At Home

You have been to your doctor. You have given him all the information about your current medications. He has prescribed you a new medication and discontinued another medication. You have asked questions to clarify exactly what each mediation is and what it is for, how to take it, what side effects are possible and how long you will be on the new medication.

 

You have been to your pharmacy to fill your prescription and update it on your list of medications. You have verified they have filled the proper medication and checked that the dosage and instructions are a match to what you were told by your doctor. You are comfortable that you understand what you need to do and what you need to be aware of and the medication is packaged in a way that you can access it.

 

Now you are home – it is up to you to stay on top of your medications or ask for help to do so if you need it! You know how important it is to take them correctly and take them at the proper time. Here are some tips for things to do once you are home:

 

  1. Open your new mediation and take the time needed to read and understand the information provided to you in the package,

  2. Add the new medication in the proper slots of your medication planner. If you do not have one, purchase one to help you track whether you have taken your medications and taken them at the right time. Use some other memory tricks to help stay on your medication schedule such as taking them at the same time every day before or after the same activity. Then, performing the other activity will actually remind you to take your medication.

  3. Be aware of the possible side effects and stay tuned in to your body. If you experience something you feel is a side effect, contact your doctor or pharmacist. Your dosage may need to be adjusted or your doctor may decide there is a different medication that will better serve you.

  4. Keep a list of your medical history and medications you take. Keep this information up-to-date. Remember to include nonprescription, herbal and dietary supplements as well. Make a copy of your list and entrust it to a family member or friend.
