ElderCare 4 Families goes the extra step to ensure our clients are receiving the best in-home care.

 

Senior Advocate the Graceful Aging Program services are included in ElderCare's hourly fee at no additional charge.



Graceful Aging Program




ElderCare's Graceful Aging Program (GAP) is another way we surpass ordinary to achieve extraordinary results.

 

Our goal is consistency and continuity of care.

 

After the Senior Advocate's initial assessment, each client and/or their family is offered a detailed plan of care, including recommendations and referrals.

 

Executing the plan of care can include all or any part of the following:

  • Determining safety concerns and fall risks in the home and attaining the best solutions (including partnering with contractors and coordinating the work)
  • Assessing any medical equipment needs and working with the physician’s office and medical equipment company or contractor
  • Researching community resources for solutions to specific needs

The Senior Advocate will continue to manage an individual’s home health care using the Graceful Aging Program (GAP). This program involves regular meetings with the Senior Advocate to precisely monitor needs and care. In addition, the Senior Advocate will routinely monitor the performance of your caregiver in the home. These visits allow us to review in-home documentation, reassess the client's care and condition, update plans of care, and communicate with approved family members or other contacts.

At ElderCare 4 Families, a Senior Advocate meets with each and every client and/or family for a complimentary assessment prior to service. This first step in the Graceful Aging Program ensures the home health care needs of your loved one are understood.

 

ElderCare's Senior Advocates are health or human services professionals dedicated to helping families care for their loved ones. This help includes coordinating care, identifying problems, recommending solutions, advocacy, and much more. The goal of a Senior Advocate is to encourage as much independence as possible while meeting the client's needs.

Senior Advocates are professionals, members of the National Association of Professional Geriatric Care Managers, and experts when it comes to the aging process. The compassion and knowledge of a home health Senior Advocate will help you best manage the growing and complex demands and responsibilities of caring for yourself or your loved one. Personalized short-term solutions and long-range plans are offered to ensure safety, independence, and a high quality of life.

 

The Senior Advocate will gather detailed information regarding your likes, dislikes, health history, family, and everyday routines. These details help us accurately select your caregiver by skill set, availability and personality matching.

 

Our goal in matching you and the caregiver is consistency and continuity of care. If for any reason and at any time, you want to modify this selection, we will immediately meet your request.
